What is this leaflet for?

This leaflet gives easy read information about the Health Promotion Team at Monkwearmouth Hospital.

They support people with learning disabilities to attend annual health checks.

We are the Learning Disability Health Promotion Team based at Monkwearmouth Hospital in Sunderland.

We support people who are attending Annual Health Checks.

The team works with other health professionals and agencies to make sure that the healthcare you receive is managed well.
