
Introduction
This leaflet provides information about the Community Learning Disability Team (CLDT). If you are not sure about anything in this leaflet please ask a member of staff.
What we do
The team works with adults who have a learning disability and are 18 years and over. We will work with you to support everything that is important for you to keep safe and well.
We can also help and support you to use other services.
How to be seen
Anybody can refer to us.
If someone is helping you to refer, then you must agree to it.
You can refer by
- phone
- letter
When a referral is made we can:
- talk to you about what we can do to help
- ask you if you are happy for us to work with you
You can say yes
You can say no
It is up to you
We will ask what help you need
If you are unsure, someone can do this for you. This is called a best interest decision.
We will listen to what you want
We can also talk to people who know you and make sure that you are involved.
We can then agree who are the best people to work with you.
The team
Speech and language therapists
help people with
• talking and listening
• understand what others say
• eating and drinking
Community nurse, assistant practitioner and support workers
carry out assessments and give
• advice
• support
• treatment
Psychologists
work with the team on assessment and treatment for lots of support needs
Physiotherapists
• check your movement and mobility
• check for risk of falls
• give help and support
Psychiatrists
are doctors who provide assessment and treatment for your mental health
Occupational therapists
assess and support your
• everyday skills and activities
• physical needs
Carer lead
- supports carers in their role
- provides training and advice
Training
Our team offers a range of training to staff teams, parents or carers who support people who have a learning disability.
How we will work with you
The team will work with you to support what is important to you to keep safe and well.
The team will also support you if you need to access other services.
The team works in lots of different places like homes, day services, hospitals and clinics.
We can
- see people on their own
- see people with their parents, paid carers and staff teams
- carry out group work
- speak to you and staff from social services, education and volunteers
We work Monday to Friday
Opening hours 9am to 5pm
We do not offer an out of hours emergency response service
