The Step Up Hub – South of Tyne is a community mental health team covering Sunderland and South Tyneside. The service provides support for your move from hospital back into the community and aims to keep you well. They provide assessment, treatment and care co-ordination to individuals with serious mental illness and complex needs both in the community and to inpatient wards.
Who is the Step Up Hub for?
The service is for men and women over the age of 18 years who require support to move back into the community.
