Professional referral form – Passenger/Access AssessmentsWe offer passenger assessments to both adults and children, who for many reasons find it challenging to get in and out of a car, travel safely, and load and stow their equipment safely (a wheelchair for example). The appointments are carried out at North East Drive Mobility, but in certain cases we are able to carry out home visits.
Referrals are accepted from Health Professionals and patients. Please complete one of the referral forms, contact us on 0191 287 5090 or email northeast.
We also offer a free information service on independent mobility, primarily related to car and adapted vehicle use as a driver or passenger.
